Fertility tests for men and women
Fertility problems can come from either partner, so we test both. At MCI, the first consultation, the ultrasound scan and the semen analysis are free.
When to get tested
NHS advice is to see a doctor if you have not become pregnant after a year of regular sex without contraception.
Women aged 36 or over should get advice sooner, because fertility falls faster from the mid-30s.

Tests for men: semen analysis
A semen analysis checks how many sperm there are, how well they move and their shape. At MCI it is free.
How to prepare
- Do not have sex or masturbate for at least 2 days before the test, and for no more than 7 days.
- Collect the whole sample into the clean container you are given. Do not use lubricant or a condom.
- If you collect the sample at home, bring it to the laboratory within an hour.
- Tell us if you have been unwell in the last three months, and about any medicines or supplements you take, as they can affect the result.
A second test is often advised, to confirm the first result.
What a semen analysis shows
These are the lower reference limits of the World Health Organization (2021), taken from men whose partners became pregnant within a year.
A result below a limit does not by itself mean you cannot become a father. Your doctor will explain your results.
| Semen volume | 1.4 ml |
|---|---|
| Sperm concentration | 16 million per ml |
| Total number of spermIn the whole sample | 39 million |
| Progressive motilitySperm swimming forward | 30% |
| Normal shapeMorphology | 4% |
| VitalityLive sperm | 54% |
If few or no sperm are found, your doctor may advise hormone blood tests, a scan of the testicles or genetic tests.
Tests for women
Your doctor will advise which of these you need. The ultrasound scan is free at MCI.
Ultrasound scan
An internal scan looks at the size and shape of the womb and at the ovaries.
AMH blood test
AMH gives an estimate of how many eggs are left in the ovaries. It can be done on any day of the cycle. It cannot show egg quality or whether you will get pregnant, but it helps your doctor choose the dose of medicines for IVF.
Hormone blood tests
FSH, LH and oestradiol show how the ovaries are working. Thyroid (TSH) and rubella immunity are often checked too.
Progesterone
A blood test that shows whether you are ovulating. When it is taken depends on how regular your periods are.
HSG (tube test)
An X-ray of the womb and fallopian tubes after a special dye is passed into the womb. It shows the shape of the womb and whether the tubes are open or blocked.
Swabs
Swabs check for infections such as chlamydia before some tests and treatments.
Bring the results of any tests you have already had. Before any other test is done, we will tell you what it costs.
Questions about tests
How much does a semen analysis cost at MCI?
It is free, together with the first consultation and the ultrasound scan.
Do both partners need tests?
Yes. Fertility problems can come from either partner, so we see and test both at the first consultation.
Can the AMH test be done on any day?
Yes. Unlike some other hormone tests, AMH can be checked on any day of the cycle.
Can we bring tests we have already had?
Yes. Bring the reports to your consultation, so your doctor can see what has already been done.
